Frailty is a decline in resilience, making it harder to recover from illness, accidents, or stress. It increases the risk of falls, disability, hospital admission, or the need for long-term care. While frailty is more common in older adults, it can affect younger people too.
The Frailty Care Project, led by Oldham South PCN, focuses on early support and intervention to help individuals maintain their health and independence. Our Frailty Care Coordinator will assess your needs and connect you to the right services, including home visits from our Advanced Nurse Practitioner if needed.
