The registration process is simple.

If possible, find your NHS number on the NHS website before beginning your registration.
Accessing someone else’s information (proxy access)
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.
To request proxy access, please come to Reception to complete our access request form.
Linked profiles in your NHS account
Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.
The NHS website has information about using linked profiles to access services for someone else.
Accepted Documents
Please bring two forms of identification – one form of personal identification and one proof of address.
Personal Identification: Passport or Identity Card, birth certificate, driving licence, marriage certificate, NHS Medical Card, student ID card.
Proof of Address: Bank statement, rent book / tenancy agreement, council tax statement, wage slip (if it includes home address), council tax statement, utility bill (dated within last 3 months), official letter (Home Office / DWP), driving licence.
We need two forms of ID. So your driving licence, for example, can't be used for both your Personal Identification and your Proof of Adress.
If you do not have any of the above documents, we should still be able to register you as long as you live within our practice boundary. Please speak to the receptionist about your situation.
