What is Frailty?

Frailty is a decline in resilience that makes it harder for individuals to recover quickly from physical or mental illness, accidents, or other stressful events.

It makes individuals more vulnerable and increases the risk of adverse outcomes such as falls, disability, admission to hospital, or the need for long-term care.

Although frailty becomes more common in older people it is not an inevitable consequence and can occur in other younger people.

Frailty Care Project

The project is a collaboration of GP practices (Hollinwood Medical Practice, Hill Top Surgery, Oldham Medical Services, Medlock Medical Practice, Quayside Medical Practice) focused on delivering improved care.

The aim of the Frailty Project is to provide early support and intervention while working with professionals in the community to help individuals maintain their health, independence and prevent further decline.

What To Expect

Our physical health is just one aspect of our overall wellbeing. The Frailty Project takes a holistic approach, addressing not only your physical health but also your mental, social and emotional needs.

Our Frailty Care Coordinator will conduct a telephone assessment to understand your needs. Based on the assessment, they will guide you to the appropriate services. Where clinical support is required, a home visit will be arranged with our Advanced Nurse Practitioner, who will work with you to develop a personalised care plan.
