The government has decided it will collect, process, analyse and disseminate your GP data for Planning and Research Direction 2021. From September 1st, 2021, patient data will be extracted from GP systems daily; data will be encrypted and pseudonymised. As this is a statutory obligation, practices are legally obliged to share this data.
You do not need to do anything if you are happy for your data to be shared in this manner.
If you as a patient want to opt out of your data being shared, apart from information used for your direct care, you have the option of registering a Type 1 Opt-out or a National Opt-out or both.
If you wish to register your opt out choice, you will need to print a letter (please see attached), sign it with your preferred option(s) and then send it back to the surgery by post or email it to hilltop@nhs.net.
If you do not have access to a computer / printer then you can request for the form to be sent to you by phoning 0300 303 5678 between Friday 9 am to 5 pm (excluding bank holidays).
Please do NOT phone the surgery if you have further questions as we wish to keep the phone lines open for patients needing direct care.
